Licensed Social Worker Job Requirements:

  • Experience as a social worker within a nursing home or rehabilitation center strongly preferred
  • Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities
  • Long Term Care Experience


Social Worker Responsibilities:

  • Plan, organize, develop and implement all required social services to meet the needs of residents in accordance with federal, state and local guidelines that govern skilled nursing facilities.
  • Performs all documentation requirements for social service documentation in a timely fashion.
  • Completes all admission assessments and evaluations
  • Works with the interdisciplinary team to create a plan of care for residents.
  • Works with and provides support to residents, families and staff.
  • Attends weekly Care Plan and Medicare meetings.
  • Receives and attempts to resolve resident grievances.
  • Coordinates resident discharge planning with all relevant departments and assists with scheduling outside services.
  • Participates in monthly team meetings such as QA and Safety.
