Key Responsibilities:

  • Oversee and manage the Social Services Department, ensuring high-quality support to residents and families.
  • Assess psychosocial needs and develop individualized care plans in collaboration with clinical teams.
  • Lead discharge planning and transitions of care.
  • Provide crisis intervention, counseling, and ongoing emotional support.
  • Ensure compliance with all federal, state, and facility regulations.
  • Supervise, train, and support social work staff.
  • Serve as a liaison between residents, families, and interdisciplinary teams.
  • Coordinate the facility’s outreach program or services to families of residents.
  • Perform advocacy services on behalf of the residents to ensure that concrete needs are met, such as clothing, laundry, and the resident's personal needs allowance if one is maintained.

Qualifications:

  • Bachelor's degree in Social Work (BSW) required; MSW preferred.
  • Minimum 2–3 years of experience in long-term care or geriatric settings.
  • Strong leadership, communication, and problem-solving skills.
  • Knowledge of state and federal LTC regulations, including MDS and care planning.
