This position delivers professional services to persons who are homeless or at risk of homelessness in a safe therapeutic manner. This position provides community based direct care, service coordination and administration of program standards to persons served in housing administered and subsidized by this program that are located within the community, specially designated section 8 vouchers, and the coordinated entry system.
Qualifications:
- Requires a Bachelors degree or equivalent life and/or professional experience particularly with housing and homelessness..
- Two years work experience in human service field preferred.
- Knowledge of the McKinney Vento Act , Illinois Domestic Violence Act and HUD Homeless guidelines or willingness to learn.
- Knowledge of the needs of women and their children who are experiencing homelessness.
- Ability to work flexible hours including some weekends and evenings
- Excellent assessment, counseling, and training skills.
- Ability to maintain the highest level of confidentiality and professionalism.
- Valid driver's license and proof of auto insurance.
Duties and Responsibilities:
- Assures program compliance with regard to HUD requirements, Vermilion County Housing Authority Section 8 program and the Homeless Prevention and Rapid Rehousing Grant and/or any other housing program/grant being administered by the program.
- Assist in the administration of the Coordinated Entry System.
- Actively remain knowledge/informed about social agencies in the area for possible referrals.
- Responsible for HUD, HMIS Data entry, DHS program data entry and record keeping. Section 8 program record keeping, and ensuring that all required grants, documentation, reports, and statistics are completed in a timely manner or other grant requirements as directed by the Director.
- Maintain appropriate records of persons served - filing of service information and preparation of new files.
- Complete a comprehensive assessment, when appropriate, which includes the person’s history, mental status, strengths, needs, abilities, preferences, and resources including recommendations for support services and obtain all necessary signature.
- Develop service plans for all residents of the permanent housing and section 8 voucher program. Service plans include statement of problem(s) and interventions/services to be provided to attain goals
- Maintain statistics necessary to compile and maintain outcome measures.
