Position Purpose
The RN Care Manager at Nazareth Hospital is responsible for coordinating and facilitating all aspects of discharge planning for an assigned patient population. This role ensures that patients receive appropriate post-acute care and support, tailored to their age-specific and cultural needs. The RN Care Manager works collaboratively with physicians, social workers, and the healthcare team to promote safe and effective transitions of care.

What You Will Do
- Conduct timely assessments of patients to identify discharge planning needs.
- Modify care planning approach based on age-specific and cultural considerations.
- Collaborate with interdisciplinary teams to develop and implement safe discharge plans.
- Advocate for continued authorization of post-acute services and obtain prior authorizations for medications.
- Refer complex cases to Social Work or Team Lead as needed.
- Maintain accurate documentation in the clinical system.
- Serve as a resource to patients, families, nursing staff, and physicians regarding discharge planning and care transitions.
- Education:
- Bachelor’s Degree in Nursing required.
- Experience:
- 2–4 years of related experience required.
- 5–8 years preferred.
- Licensure/Certification:
- Active PA RN License required.
- Preferred Certifications:
- Certified Case Manager (CCM)
- Accredited Case Manager (ACM)
