We are seeking a dedicated professional to join our team. The position involves working shifts from 8:00 AM to 4:30 PM, five days a week. The role includes weekend rotation every 7 weeks, and holiday coverage as needed. There is no floating requirement for this position.

Requirements

• Acceptable candidates include RN Case Manager Discharge Planners or Licensed Social Workers.
• Social workers must be eligible for LMSW/LGSW/LCSW and must have passed the necessary exam.
• DC RN Licensure is required. We will accept pending DC licensure with proof of application, or the candidate can hold LMSW/LGSW/LCSW.
• BLS certification is required.
• A minimum of 2 years of experience is required.
• Candidates must have acute hospital discharge planning experience.
• Required skills include experience in Emergency Department and Case Management.
