Regional Social Worker
Job Summary:
Under the supervision of the Corporate Director of Social Services, the Regional Social Worker supports the psychosocial and emotional well-being of residents across multiple skilled nursing facilities within the Optalis network. This leadership role provides clinical support, training, oversight, and regulatory guidance to facility-level social workers, ensuring compliance with federal, state, and company policies. The Regional Social Worker partners closely with facility staff, administrators, interdisciplinary teams, and families to promote quality of life, resident advocacy, and smooth transitions of care.
Essential Job Duties & Responsibilities:
- Provide onsite and remote support to facility social workers, ensuring consistent adherence to federal and state regulations (CMS, OBRA, HIPAA).
- Conduct routine audits of resident records and social service assessments and documentation to ensure accuracy and compliance.
- Support social workers with complex discharge planning, behavioral intervention planning, and crisis resolution.
- Assist with mock surveys as requested by Corporate Director of Social Work.
- Train and mentor new and existing social work staff; develop training tools and best practice guidelines.
- Provide coverage to facility in all aspects of Social Work staffing needs in absence of Director of Social work, when staffing is critical, or as assigned by Corporate Director of Social Work.
- Support facilities during regulatory surveys and assist with developing and implementing plans of correction.
- Facilitate collaboration between social work and interdisciplinary teams to enhance resident outcomes.
- Support admission and discharge processes to reduce re-hospitalizations and ensure continuity of care.
- Monitor and guide implementation of trauma-informed care, person-centered planning, and culturally competent services.
- Travel to assigned facilities regularly and as needed to provide hands-on guidance and support.
- Attend monthly Social Service Director meetings (virtual and in-person) as scheduled by Corporate Director of Social Work.
Work Conditions:
- Frequent travel (~80%) within assigned region.
- Hybrid work model: field visits, virtual support, and occasional corporate and director meetings.
- Ability to respond to urgent matters outside of traditional work hours as needed.

Qualifications:
- Education: Master’s Degree in Social Work (MSW); Minimum, Limited License Master of Social Work.
- Experience: Minimum 5 years of experience in long-term care/skilled nursing, with at least 2 years in a regional or supervisory capacity.
- Knowledge: Strong understanding of SNF regulatory compliance, discharge planning, behavioral health needs of the geriatric population, and community resources.
- Skills: Exceptional communication, documentation, and organizational skills; ability to lead with compassion and confidence; strong working knowledge of Resident Assessment Instrument Manual, Minium Data Set completion & process, and State Operations Manual related to Social Work.
- Technology: Proficiency in electronic medical records (EMR), Microsoft Office Suite or related software, and ability to train others on system use.
