The successful candidate will have at least 2 years of working experience as a Social Worker and a minimum of 1 year working in a Skilled Nursing Home. Responsibilities:

• Counseling residents and family members; facilitating meetings
• Complete Admissions, Psycho-social Assessments, and Advanced Directives
• Make referrals for health services and coordinate discharge planning
• Completing all required documentation for medical records

• Must have at least 2 years of experience working as a Social Worker.

. Degree in Social Work.

. Skilled Nursing Home experience is a MUST!

• Should have a good working knowledge of available social services and other community resources in the area
• Must have excellent communication and social skills
