Job Description
Under direction of the Care Coordination Director, the Care Coordination Assistant Director will structure and lead daily activities for the assigned team focused on serving high risk behavioral health patients (including Health Home, New York City Department of Health and Mental Hygiene (DoHMH) grant/Non-Medicaid, Health Home Plus, Assisted Outpatient Therapy/AOT, and/or Health and Recovery Plan [HARP], etc.).
Minimum Qualifications
Valid license and current registration to practice as a Licensed Clinical Social Worker (LCSW) issued by the New York State Education Department (NYSED); and one (1) year of supervisory experience utilizing casework, group work and/or community organization methodologies.
Department Preferences
Knowledgeable In
Health Home and/or other Care Coordination Services.
Willingness and ability to regularly travel between assigned work sites.
Computer Programs/Software Operated
Proficiency/Experience in the following:
Minimum of 1-3 years’ experience in successfully supervising line and supervisory personnel

- Electronic Health Record (EHR) System
Minimum of 1-3 years’ experience in successfully supervising line and supervisory personnel
- Minimum of 2-4 years’ experience in care coordination
