About Job

Position: Medical Social WorkerLocation: Evanston HospitalFull Time/Part Time: Full timeHours: Monday-Friday, 8am – 5pmRequired Travel: yesResponsibilities:- Performs psychosocial assessments with patient and family consistent with age and developmental status.- Communicates care issues identified through assessments to healthcare team.- Develops social work intervention plans based on assessments.- Provides counseling to patients and families adjusting to illness, disability, or loss.- Assists patients and families in crisis with emotional support and active listening.- Educates patients and families on decision-making rights such as advance directives.- Provides assessment, crisis intervention, and referrals to protective services per Illinois law.- Educates on community resources for health maintenance and psychological support.- Performs age-specific discharge planning involving patients and families.- Documents all assessments, plans, and actions in medical records per standards.Requirements:- Masters Degree from accredited school of social work.- LSW or LCSW license.- Preferred 2 years post-graduate experience in healthcare or mental health.- Skills: healthcare environment experience, teamwork, knowledge of abuse/neglect laws, crisis intervention, Medicare/Medicaid knowledge, social work ethics, confidentiality, developmental issues, diversity.Benefits:- Premium pay for eligible employees- Career growth pathways- Medical, dental, pet, vision options- Tuition reimbursement- Free parking- Wellness program- Health savings account options- Retirement with company match- Paid time off and holiday pay- Community involvement opportunitiesAbout the company:Endeavor Health is a fully integrated healthcare system serving over 4.2 million residents in northeast Illinois with more than 25,000 team members and 6,000 physicians across multiple hospitals and ambulatory locations.
