Overview:

Fortcom Health Services is seeking a Per Diem Care Manager to support our Adult Foster Care (AFC) program. This flexible role is ideal for a licensed social worker looking to make a meaningful impact through community-based care while maintaining a work schedule that fits their availability. The Per Diem Care Manager will conduct member home visits, complete documentation, and collaborate with the care team to ensure members receive appropriate, person-centered support.

Key Responsibilities:



  • Conduct initial and ongoing psychosocial evaluations of AFC members.
  • Assess and document the safety and suitability of the home setting.
  • Participate in the development, implementation, and review of individualized AFC care plans.
  • Conduct scheduled home visits and complete detailed care management progress notes.
  • Review AFC caregiver logs at least every 30 days.
  • Assist with training, evaluating, and supporting AFC caregivers in collaboration with the RN.
  • Coordinate care and services with other healthcare and community agencies (e.g., VNA, DDS, DMH, MRC).
  • Respond promptly to changes in member condition and communicate with the AFC RN.
  • Assist with discharge and transition planning as needed.
  • Maintain accurate and timely documentation in accordance with program requirements.
Qualifications:

  • Bachelor’s degree in social work, psychology, human services, or related field.
  • Licensed Social Worker (LSW or higher) in Massachusetts.
  • Previous clinical experience working with elderly adults or individuals with disabilities.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work independently while contributing to a collaborative care team.
  • Valid driver’s license and reliable transportation.
Job Conditions:

  • Must be able to travel to member homes across the assigned area.
  • Ability to conduct home visits throughout assigned regions.
  • Must be able to access homes that may not be wheelchair accessible.
  • Requires the ability to walk, sit, stand, stoop, and occasionally lift or support members during visits.
