Overview:
Fortcom Health Services is seeking a Per Diem Care Manager to support our Adult Foster Care (AFC) program. This flexible role is ideal for a licensed social worker looking to make a meaningful impact through community-based care while maintaining a work schedule that fits their availability. The Per Diem Care Manager will conduct member home visits, complete documentation, and collaborate with the care team to ensure members receive appropriate, person-centered support.
Key Responsibilities:
- Conduct initial and ongoing psychosocial evaluations of AFC members.
- Assess and document the safety and suitability of the home setting.
- Participate in the development, implementation, and review of individualized AFC care plans.
- Conduct scheduled home visits and complete detailed care management progress notes.
- Review AFC caregiver logs at least every 30 days.
- Assist with training, evaluating, and supporting AFC caregivers in collaboration with the RN.
- Coordinate care and services with other healthcare and community agencies (e.g., VNA, DDS, DMH, MRC).
- Respond promptly to changes in member condition and communicate with the AFC RN.
- Assist with discharge and transition planning as needed.
- Maintain accurate and timely documentation in accordance with program requirements.
- Bachelor’s degree in social work, psychology, human services, or related field.
- Licensed Social Worker (LSW or higher) in Massachusetts.
- Previous clinical experience working with elderly adults or individuals with disabilities.
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently while contributing to a collaborative care team.
- Valid driver’s license and reliable transportation.
- Must be able to travel to member homes across the assigned area.
- Ability to conduct home visits throughout assigned regions.
- Must be able to access homes that may not be wheelchair accessible.
- Requires the ability to walk, sit, stand, stoop, and occasionally lift or support members during visits.
