Job Description

Regions Hospital is seeking an experienced RN to work as an RN Care Manager role in the ED. This is a part time, benefit eligible position with evening shifts and every other weekend. Evening shifts from 1300-2130, weekend shifts are 12 hours 0900-2130.

This position provides inpatient vs Hospital care management services for medical/surgical patients to facilitate optimal patient outcomes, supports the delivery of effective and efficient care, improve continuity of care and efficient use of resources to progress the plan of care. Focuses on developing a discharge plan to prevent readmissions and patient focused. The RN Case Manager will collaborate with the care team including, but not limited to: attending/specialty physician(s), Social Work, Utilization Review, therapies, and Nursing to ensure that an effective and timely treatment plan is acted upon, and appropriate discharge plan is implemented with a smooth transition in levels of care. Provides Care Management services on weekends and/or holiday(s) rotations as determined by leadership.

The ED Care Managers work collaboratively with all hospital departments and external partners to facilitate patient flow and coordination of other services beyond Emergency Room, such as home care, SNF/TCU, resources or outpatient services. ED Care Managers have an increased focus on hospital avoidance, frequency of ED returns and identifying patient triggers that could lead to readmission or return.

Work Schedule:

Evening shifts from 1300-2130, weekend shifts are 12 hours 0900-2130. Current shift is every other weekend, can explore every third weekend. this is a Pay Period Averaging position, so hours vary by week with overall 0.6 FTE

Required Qualifications:
  • Associate's degree of nursing with RN licensure from an accredited college
  • 5 years' experience working as an RN Care Manager or 3 years working as RN Care Manager within HealthPartners organizations.
  • Current State of Minnesota Nursing Board RN license

Preferred Qualifications:
  • Bachelors or Master's degree of Nursing
  • Three (3) years' experience. Experience performing, case management in Home Care, hospital or Post-Acute Care, discharge planning, care and/or quality improvement.
  • Certified Case Manager through Commission for Case Manager Certification (CCMC) or eligibility to sit for CCM exam within 2 years of hire.
