Position Overview

The Five Rivers Health Centers Care Coordinator rolesupports the mission and vision of the organization by performing outreach and assisting with the coordination and/or tracking of care. Care coordinators will be tasked with:

  • Addressing transitions of care.
  • Identifying and assisting with closing care gaps.
  • Tracking emergency room (ER) visits and hospital admissions.
  • Coordinating incoming clinical documentation to be included in the electronic health record.
  • Conducting outreach to patients overdue or almost overdue for screenings, visits, and/or immunizations.
  • Scheduling follow-up appointments.
  • Tracking outreach activities from Care Message and Azara platforms.
  • May participate in other quality-related functions as appropriate.

EXPERIENCE, SKILLS, AND EDUCATION

  • High School diploma or equivalent. (Required)
  • Associate’s degree in healthcare or a business-related field. (Preferred)
  • Healthcare experience. (Preferred)
  • Proficiency with Microsoft Word and Excel. (Required)
  • Medical Terminology knowledge. (Preferred)
