Job Overview
The Nurse Care Manager, also known as an Aging Life Care Manager, specialize in assisting older adults and their families navigate the challenges of aging. They offer a holistic, client-centered approach, providing support and guidance in areas like healthcare, housing, finances, and legal matters. They act as advocates, care coordinators, and educators, helping families navigate the complexities of aging and ensure their loved ones receive the best possible care. The Nurse Care Manager plays a pivotal role in coordinating and managing patient care across the home and clinical settings, ensuring that patients receive comprehensive and effective treatment. This position is an independent contractor position managing a case load of 10-12 clients, as well as occasional on-call duties. The Nurse Care Manager should be a strong communicator and knowledgeable about local resources to help seniors age in place safely.

Duties

  • Conduct thorough patient assessments to determine healthcare needs and develop individualized care plans.
  • Coordinate care across multiple settings including inpatient, outpatient, post-acute care, and home environments.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient service and continuity of care.
  • Monitor patient progress and adjust care plans as necessary based on clinical evaluations and patient feedback.
  • Facilitate discharge planning and ensure proper follow-up care is arranged for patients transitioning out of acute settings.
  • Maintain accurate medical records in compliance with HIPAA regulations and ensure confidentiality of patient information.
  • Provide education to patients and families regarding treatment options, health management, and available resources.
  • Gather current information on and recommend local home care companies, caregivers, assisted living facilities, memory care facilities, etc.
  • Engage in utilization management to optimize resource use while maintaining high standards of patient care.

Qualifications

  • Care management experience preferred.
  • Registered Nurse or higher degree.
  • Familiarity with geriatrics, med-surg, and life care planning a plus.
  • Flexibility to adjust schedule or visits, based on acute client needs.
  • Strong understanding of patient assessment techniques and vital sign monitoring is necessary for effective patient management.
  • Previous experience in outpatient clinic settings or managed care environments is a plus.
  • Excellent communication skills are essential for effective collaboration with healthcare professionals and patients alike.

Pay: $70.00 - $85.00 per hour

Expected hours: 15 – 40 per week
