We’re hiring a dedicated SUD Care Coordinator to guide individuals toward recovery and stability. Must hold a licensure as a Certified Master's Level Social Worker in Idaho—bring your expertise where it’s needed most.
The Substance Use Disorder (SUD) Care Coordinator provides expertise focused on supporting individuals who have substance/opioid use disorder by identifying resources, developing care plans, and coordinating services to meet their needs. This position will act as a liaison, advocate, and facilitator to ensure individuals access necessary support and achieve their goals. The Care Coordinator will be expected to participate in overall program planning, implementation, and management of funding within defined guidelines within the objectives of the Opioid Settlement Funds (OSF) and other funders to execute deliverables.
Qualifications and Proficiencies:
- Licensure as a Certified Master's Level Social Worker in Idaho in accordance with Idaho Code 54-32. A licensure or temporary permit must be obtained prior to appointment.
- Considerable knowledge of Opioid use disorder, other substance use, and mental health disorders, and their required treatment and rehabilitation, their etiology, and basic diagnostic and assessment methods
- Good knowledge of community mental and behavioral health models of service delivery is necessary to provide appropriate levels of care and treatment as an alternative to hospitalization or incarceration; management principles and practices
- Experience planning, coordinating, and evaluating social service treatment programs for those with OUD and any co-occurring SUD/MH disorders
- Understanding of computer programs and technologies essential to maintaining program effectiveness and efficiency.
- Knowledge of documentation requirements, electronic health records, State and Federal laws that govern the release of health care information, and patient consent.
- Valid driver's license (Idaho or Washington) or ability to obtain one if relocating.
- National Incident Management System (NIMS)/Incident Command System (IS)100, 700 within one (1) month of employment.
Physical Requirements:
Due to the need for all Health Department personnel to respond to public health emergencies, the employee must be assessed for his/her ability to meet the physical demands of performing the following activities:
- Engage in the following movements: climbing, balancing, stooping, kneeling, crouching, reaching, standing, walking, lifting, fingering, grasping, feeling, and writing and repetitive motions.
- To move objects, exert up to 30 pounds of force occasionally and/or a negligible amount of force constantly.
- Hearing ability sufficient to communicate effectively in person and over the phone.
- Visual ability must be sufficient to read typewritten documents, computer screens, and drive a car
