Mountain View Hospital is looking for Discharge Planners to join their team!

Shift does include rotating weekends and holidays. Both Full-Time and Part-Time available
Responsibilities include but are not limited too:
- Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem.
- Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations.
- Prepares patient histories, service plans, and reports
- Participates in planning for improving health services by interpreting social factors pertinent to development of program.

Qualifications
- RN or LPN required.
- Experience in discharge planning and case management is a plus
- Knowledge of medical terminology and HIPAA regulations
- Familiarity with acute care settings and medical records management
- Preferred experience in hospital settings and medical coding (CPT, ICD-9, ICD-10)
Starting wage will increase based on qualified experience and license type.
