Job Title: Hospice Social Worker

Reports to: Director of Support Services

Department: Support Services

Job Summary:

The Hospice Social Worker is responsible for furthering the mission of Minnesota Hospice by providing clinical support to patients and their families in conjunction with the other members of the care team. The Social Worker has specialized experience related to the psychosocial, spiritual, bereavement, environmental and financial needs of patients and their families.

Duties/Responsibilities:

· Provide psychosocial care of patients and families in accordance with Medicare guidelines

· Perform psychosocial assessments within 5 days of admission

· Develops and completes a Plan of Care (POC) no less than every 15 calendar days

· Provide support and counseling to the patient and family within the Social Worker scope of practice

· Advocate for patients and families to maximize psychosocial support within the scope of practice

· Communicate, coordinate and collaborate with team members to plan and implement the Plan of Care (POC)

· Facilitate family conferences as necessary

· Complete financial disclosure form with patient or family and provide financial advocacy and counseling as appropriate

· Maintain current and confidential medical records in accordance with Medicare guidelines

· Participate in bi-weekly Interdisciplinary Team (IDT) meetings to facilitate and coordinate the psychosocial components of the Plan of Care (POC)

· Coordinate discharge planning with facility personnel, facilitating discussion regarding any grievances between Minnesota Hospice and facility personnel

· Facilitate funeral planning as appropriate

· Communicate, coordinate, and collaborate with facility personnel, offering support to grieving staff as appropriate

· Maintain knowledge of appropriate community resources and act as a resource to the hospice team on psychosocial issues and community resources

· Act as a liaison to community resources for patients and their families

· Assist with Advance Directives including living wills, POLST, etc.

· Assist with and facilitate Medicaid applications as well as other care programs

· Act as a liaison to religious/spiritual institutions and support spiritual care

· Act as a resource and support person to caregiving volunteers

· Maintain appropriate caseload based on the number of scheduled work hours

· Participate in Minnesota Hospice sponsored educational in-services, staff meetings, and agency events

· Perform Social Work services in compliance with Minnesota Hospice policies and procedures and the NASW Code of Ethics

· Provide bereavement support and counseling to designated primary bereaved as needed

· Present about hospice services to community groups as requested

· Other duties as assigned

Required Skills/Abilities:

· Knowledge of appropriate HIPAA and confidentiality

· Excellent oral and written communication skills

· Proven ability to handle confidential information with minimal supervision

· Ability to function independently

· Able to work as both a part of a team and independently

· Possess great organizational skills

· Strong decision making and problem solving skills

· Excellent time management and prioritization skills

· Proficient in word processing and computer skills in addition to the use of automated equipment and software

Education and Experience:

· LSW required, MSW preferred

· Current licensure through the MN Board of Social Work

· Minimum of 1 year related experience in healthcare setting

· One year hospice or long term care experience preferred

Physical Requirements:

· Able to regularly lift and move up to 20lbs

· May have prolonged periods of sitting at a desk and working on a computer

· Personal protective equipment provided, including masks

Job Type: Full-time

Pay: $65,000.00 - $80,000.00 per year
