DEPARTMENT: Oncology Service Line Administration
General Summary
The Lead Oncology Social Worker is an experienced clinician and key member of the multidisciplinary oncology team. This role provides emotional, psychosocial, and practical support to cancer patients and families, manages the MSW Internship Program, and serves as a consultant to clinical staff regarding psychosocial care and community resources.
Essential Duties And Responsibilities
- Provide counseling and support to patients and families throughout the cancer journey.
- Serve as a clinical role model, demonstrating best practices and ensuring quality care.
- Research, develop, and implement innovative psychosocial support programs for cancer survivors.
- Participate in identifying patient/family psychosocial needs in collaboration with the Cancer Committee and medical staff.
- Facilitate general cancer support groups and participate in psychosocial events and outreach activities.
- Identify complex discharge needs and coordinate individualized care plans.
- Lead multidisciplinary meetings to address complex patient cases and discharge planning.
- Consult with Case Coordinators on inpatient discharge needs and make appropriate post-hospital care recommendations.
- Document all patient interactions and services provided in the medical record.
- Coordinate grant-funded transportation services for eligible patients.
- Liaise with external providers and insurers to secure appropriate post-treatment care.
- Promote effective communication and transition between care providers and services.
- Refer patients to appropriate protective or legal services based on age-specific needs.
- Connect elderly patients with community resources to support independence.
- Assist patients with financial concerns including accessing pro bono care, insurance, and disability services.
- Refer complex cases regarding medical necessity to the Clinical Resource Management Committee.
- Conduct staff education sessions related to oncology social work.
- Adhere to hospital infection control and safety protocols.
- Participate in hospital committees, quality improvement efforts, and professional development.
- Research and implement best practices to enhance service delivery in Case Coordination.
Skills & Abilities:
- Strong communication and collaboration skills.
- Research and evaluation expertise.
- In-depth knowledge of local community resources and age-specific care needs.
- Proficiency in hospital information systems.
- Ability to work flexible hours and navigate hospital environments.
- Commitment to patient confidentiality and professional ethics.
- Master’s degree in Social Work (MSW) from an accredited program.
- Licensed Social Worker in the District of Columbia.
- 4–8 years of healthcare social work experience preferred.
- Current CPR certification required.
- Reports to: Senior Coordinator, Sibley Center for Patient and Family Services
- Supervises: MSW Interns
