Position Summary:
We are seeking a dedicated and experienced RN Care Manager to join our team. In this role, you’ll use your clinical expertise to oversee client care, support our caregivers, and ensure the highest standards of service. Key Responsibilities:

• Develop and update individualized care plans tailored to client needs
• Conduct initial in-home assessments and ongoing reassessments / client care visits to ensure quality of care
• Set up and manage weekly medication regimens for clients
• Supervise, support, and provide guidance to caregiving staff
• Serve as a liaison between clients, families, caregivers, and healthcare providers
• Provide hands on care to client’s as needed
• Complete and submit documentation required by Long-Term Care Insurance (LTCI) providers
• Ensure compliance with company policies, state regulations, and best practices in home care
• Take on call phone for after hours when scheduled

Qualifications:

• Active and unencumbered RN license in the state of Arizona
• Strong organizational and clinical assessment skills
• Excellent communication and leadership abilities
• Familiarity with Long-Term Care Insurance documentation is a plus
• Prior experience in home care, care management, or case management preferred
• Flexible on weekly schedule as it may rotate from time to time

Pay: $70,000.00 - $75,000.00 per year
