We offer geriatric care management services, non-medical home care services, and RN/LVN home health services to clients who are billed on a private-pay basis. Work Schedule: On call hours Fridays at 4:30pm through Mondays at 9:00am, with some availability for as-needed client visits preferred. Some flexibility is possible - if this schedule doesn’t exactly match up with your needs please don’t let that stop you from applying! On call hours are performed remotely, with client visits performed in-person in San Diego County as care needs dictate.

Pay: $33.00 - $37.00 per hour plus on call stipend

Responsibilities include:

• Managing crisis situations
• Handling urgent client and staff concerns
• Overseeing and assuring quality of care provided to clients after hours
• Orienting Home Care Aides to new client needs
• Completing skilled nursing visits in 1:1 settings, including medication management, injections, ostomy and catheter care, g-tube feedings and other nursing procedures as clinically necessary
• Assessing and managing client risk
• Partnering with On Call Staffing Coordinator to communicate staffing and schedule changes to clients and involved parties
• Advocating for clients currently in home, facility and medical settings

Desired knowledge, skills & experience include:

• Active California Vocational Nurse License (LVN)
• 1+ years of paid clinical nursing experience with adults who are older and/or have physical and/or mental disabilities
• Excellent written and verbal communication skills with attention to detail
• Working knowledge of the provision of physical and mental health care in a variety of settings
• Working knowledge of care resources, insurance and care delivery systems in San Diego County
• Moderate+ technology proficiency, including use of cell phone apps and web-based office systems such as Google Workspace
• Ability to work independently and manage multiple priorities in a fast-paced environment, and report mileage, expenses, and billable hours accurately
• Active driver’s license, clean driving record, and reliable vehicle
• Current CPR certification
• Ability to pass a drug screening and health assessment
• Ability to pass a Home Care Services Bureau (HCSB) background screen and TB test
• Ability to lift, push or pull up to 40 lbs. unassisted
