Summary
As a Care Manager at Elder Options, you will play a vital role in coordinating and overseeing the care of older adults in their homes. This position is essential in ensuring that clients receive personalized support tailored to their unique needs, enhancing their quality of life and independence.
Responsibilities
- Conduct comprehensive assessments of clients' needs to develop personalized care plans.
- Coordinate with healthcare providers, family members, and caregivers to ensure effective communication and service delivery.
- Monitor client progress and adjust care plans as necessary to meet evolving needs.
- Provide guidance and support to caregivers, ensuring they have the resources needed for effective care.
- Educate clients and families about available services and resources within the community.
- Maintain accurate records of client interactions, assessments, and progress reports.
- Coordination with care providers to ensure concerns/issues are addressed timely.
- Crisis intervention.
Requirements
- Bachelor's degree
- Excellent communication skills for effective interaction with clients, families, and healthcare professionals.
- Ability to work independently while managing multiple cases effectively.
- Compassionate, empathetic, attentive, and task orientated individual.
- Attention to detail and ability to "think outside of the box".

*Pay is $25.00 for administrative time and $28-$32 for billable time, based on experience. Billable caseload fluctuates but, expectation is at least 1/3 of time in the office is spent on billable activities. For example, 3 days in the office (7a-4pm with 1/2 lunch = 7.5 hours). 3 office days = 22.5 hours. Billable expectation would be at least 7.50 hours/week, which would be paid at the "billable rate".
