Job Summary

We are looking for a positive and energetic person to join our team as our
Resident Care Coordinator (RCC). The RCC is responsible for providing oversight of the resident health care service needs. They oversee the medication delivery system and coordinate training for the health services department. As an RCC, you will be able to have a positive impact on the lives of your team of caregivers as well as our residents and family members.

Responsibilities

  • Facilitates the overall coordination of care for residents and provides oversight for the health services department staff in accordance to the Health Service Manual.
  • Participates in the pre-move in assessment, review of move in paperwork and initial service plan development.
  • Participates in routine review of resident service plans to identify needs for additional assessment, health monitoring, or other coordination of care needs.
  • Coordinates residents health care in collaboration with outside health care agencies ie pharmacy, physician, home health, hospice etc.
  • Verifies completion of job duty orientation and training through routine review of the training documents. Coordinates continuing training and education in accordance with state and facility policy.
  • Conducts Health Services related audits in accordance with community QA program.
  • Participates in the recruitment, performance evaluation and disciplinary action of health services department staff.

Qualifications

  • Demonstrates knowledge and utilization of basic management skills.
  • Is a self-starter and requires minimal supervision
  • Organizes and utilizes time appropriately, sets priorities and accomplishes assigned tasks.
  • Displays mature behavior in attitude and speech and action and demonstrates consistently pleasant demeanor, tone of voice, supports the philosophy and approach used by the facility.
  • Works cooperatively with supervisors and co-workers.
  • Is accurate, dependable, neat and thorough in assigned tasks.
  • Is effective in both verbal and oral communication

Job Type: Full-time Tuesday - Saturday, with on call and possible shift coverage.
