Job Summary
The CCHIP Integrated Care Coordinator (ICC) will work regionally across Cook County HIV Integrated Programs (CCHIP) to assist chronically complex HIV patients to address their specific medical/behavioral needs by managing a comprehensive external referral system, under the direction of the Cook County HIV Integrated Programs (CCHIP) Program Director and direct supervision of the CCHIP Program Manager (PM). The ICC will assist the Provident and SSHARC Program Manager in the management of the Population Centered Health Home (PCHH) Healthcare Access program. The ICC will deliver population-based care support to CCHIP staff to ensure no client falls out of care along the HIV Care Continuum. The primary goal of these clinical and essential support referrals will be to promote the use of antiretroviral’s (ARV) and medical treatment. The ICC provides advice and assistance to CCHIP multidisciplinary team members who will assist clients in obtaining medical, social, community, legal, financial, and other needed non-medical case management services (NMCM) services. The ICC will collaborate with the CCHIP Specialty Care Coordinator to manage Population Centered Health Home (PCHH) Domain 4: Referral Services and linkage agreements. The ICC directly supervises program staff and assists with community HIV testing coordination, HIV linkage and re-engagement to HIV primary care as needed. This position is grant funded until February 28, 2026
Typical Duties
- Works alongside the CCHIP PM to coordinate the activities of the Consumer Advisory Board (CAB). Works alongside the CCHIP Consumer Development and Advocacy Coordinator to ensure Provident patients participate in CCHIP Joint CAB meetings.
- Co-Leads the Provident Quality Improvement meetings alongside the PM and Data Manager. Attends monthly CCHIP CQI meetings at the RMR CORE Center. Works collaboratively with the PM and CCHIP Quality Improvement Coordinator on continuous quality improvement (CQI) projects and activities.
- Assists PM in preparing check requests, invoices, purchase requisitions and other daily operational tasks and other administrative duties as assigned.
- Works alongside the PM on daily staff activities at the SSHARC and Provident CCH location. In the absence of the PM, provides direct oversight as needed. Collaboration with other CCHIP Domain Leaders, including domain 2 co-leadership and regular attendance to all three domain meetings and other CCHIP meetings.
- Works alongside the CCHIP Program Director to ensure client eligibility for CCHIP NMCM services. Communicate with CCHIP data team to review documentation of non-medical case management services in CAREWARE and the CCH electronic medical record (EMR)
- Prepares documents for general communication, including meeting announcements, minutes from case conferences and staff meetings.
- Monitors Provident provider schedules and assist with multidisciplinary case conferences as needed.
- Works Alongside Regional PrEP Coordinator to ensure all prevention staff receive training on PrEP, EIS activities, including but not limited to HIV testing, health education and literacy training, referrals and community outreach/events.
- Assists with coordinating and attending community events promoting HIV awareness, testing, and counseling.
- Facilitates CCHIP wide training of internal and external referrals and how to deliver non-medical case management services at domain meetings.
- Provides direct supervision to program staff to ensure linkage to care and re-engagement activities to promote retention in care. Ensure proper documentation of EIS/Outreach services in EMR, PROVIDE, CAREWARE.
- Assists with grant writing as needed.
- Promotes HIV program services and collaborate with community partners, identify new locations for sexual health service delivery, (inclusive of HIV Testing, PrEP, condom distribution. Works alongside the CCHIP Specialty Care Coordinator to assist with new and upcoming renewals of linkage agreements.
- Scans Ryan White Eligibility documents into PROVIDE.
- Performs other Duties as assigned.
Minimum Qualifications
- Bachelor’s degree from an accredited college or university is required (Official transcripts are due at the time of interview)
- Two (2) years of experience working with HIV/AIDs clients and/or with diverse and complex clients including LGBTQIA population is required
- One (1) year of experience collaborating with the community and performing outreach is required
- One (1) year of supervisory and/or managerial experience is required
- Proficiency using Microsoft Office is required
- Position requires moderate travel for which the employee must have a valid driver’s license in good standing and access to an insured vehicle is required
- Must be able to work flexible hours outside of a usual day shift schedule that may include evenings and weekends is required
Preferred Qualifications
- Two (2) years of related experience is preferred
- Experience working in a hospital, social service or outpatient ambulatory setting is preferred
- Intermediate proficiency using Microsoft Office is preferred
