Job Overview
We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team. The ideal candidate will possess a strong background in case management and experience in hospital discharge process. Providing specialized support for individuals and families facing the challenges of kidney disease, dialysis, and transplantation. Experience is preferred but willing to train. This role requires expertise in diagnostic evaluation, crisis management, benefits coordination and patient assessment, with a focus on delivering high-quality services and offering psychosocial assessments, counseling, and support services to help patients cope with the emotional, social, and practical implications of their condition.

Responsibilities

  • Psychosocial Assessments: Evaluating patients' emotional, social, and financial well-being to identify areas of need and develop individualized support plans.
  • Counseling and Support: Providing emotional support, crisis intervention, and coping strategies to help patients adjust to their diagnosis, treatment, and lifestyle changes.
  • Education and Advocacy: Educating patients and families about kidney disease, dialysis, and transplantation, as well as advocating for their rights and needs within the healthcare system.
  • Case Management: Assisting patients with navigating the complex healthcare system, accessing resources and services, and coordinating care with other healthcare professionals.
  • Interdisciplinary Team Collaboration: Working with physicians, nurses, dietitians, and other healthcare professionals to provide comprehensive patient care.
  • Family Support: Providing support and resources to families affected by kidney disease, including children, spouses, and caregivers.
  • End-of-Life Care: Assisting patients and families with end-of-life decisions and advance care planning.
