Social Services Manager

Hudson Elms is a 50 bed skilled nursing, and 25 bed Assisted Living Community looking for a Experienced Social Worker in Long Term Care or Behavioral Health, who enjoys working with residents in the community. The ideal candidate will be responsible for the psycho-social needs of our resident, and manage the coordination on ancillary services, assessments and discharge planning.

Job Responsibilities:

• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (physical, cognitive, and socialization factors) in planning social services programs.
• Provides orientation to long term care facility and services for residents and family members
• Promptly follows-up on admission inquiries, evaluates and assess the needs of prospective resident to determine appropriateness of facility placement
• Schedules appointments with resident, responsible party, guardian, next of kin or agency to;
  • Review Bill of rights, authorization for medical records, Patient Care Conference Meetings (etc.)
• Notifies all disciplines if resident is recipient of Medicaid funding if resident’s condition does not warrant a return within 15-20 day period.
• Conducts discharge planning conference at the discretion of the planner, issues pending discharge bulletin, and formulates post-discharge summary.
• Completes Social History after conference with resident, family and/or responsible party withing 14 days of admission
• Completes the initial psychosocial assessment section of Social History, and Social Service plan withing 14 days of admission

• Ensures all social service assessments and progress notes are up to day on the resident medical chart in a timely basis, and at least quarterly, completes a progressive assessment note.

Summary of Qualifications:

• One to two years of experience in a related field preferred
