What You’ll Do:
- Perform assessments, develop care plans, and conduct skilled nursing visits
- Serve as liaison between care providers, clients, and family members
- Educate clients and caregivers on chronic disease management and safety
- Ensure full compliance with OASIS and state/federal regulations
Qualifications:
- Current MN RN license
- Minimum 3 years of RN experience
- Familiarity with home care documentation and regulations
- Valid driver’s license and reliable vehicle
