Job Summary:
Comfort & Care Home Services, LLC is seeking a dedicated Licensed Clinical Social Worker, Licensed Professional Counselor or License Eligible to join our team in Virginia. The ideal candidate will be responsible for providing outpatient counseling services to individuals in need. As a Licensed Professional, you will conduct patient assessments, relationship building and ensure the highest level of direct session care for individuals with various mental health conditions.

Duties:
- Conduct thorough data collection and patient assessments to develop personalized treatment plans.
- Provide mental health counseling and support to individuals with diverse needs.
- Collaborate with medical professionals to offer psychopharmacology assistance when necessary.
-Provide documentation of each session.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Coordinate care plans with hospitals and other healthcare systems for seamless patient support.

Qualifications:
- Must hold a valid license as a Clinical Social Worker or LPC in Virginia.

-Can be in residency.

- Proficiency in medical terminology and understanding of mental health systems.

- Strong interpersonal skills and the ability to work effectively with a diverse range of patients.

- Excellent communication skills and the ability to collaborate with multidisciplinary teams.
