Job Title: LPN Case Manager

Job Summary:

We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) Case Manager to join our care tream. The LPN Case Manager is responsible for coordinating patient care, assessing needs, developing care plans, and ensuring quality services are provided in a cost-effective manner. This role works closely with physicians, RNs, social workers, and other care team members to facilitate optimal patient outcomes.

Key Responsibilities:

Conduct resident assessments and assist in developing individualized care plans
Monitor resident progress and update care plans as needed
Coordinate healthcare services and transportation to medical appointments and ensure compliance with treatment plans
Communicate regularly with physicians, specialists, insurance, and healthcare providers
Maintain accurate, up-to-date resident records and documentation
Identify and address barriers to care and escalate concerns appropriately
Adhere to all regulatory requirements and company policies

Qualifications:

Current and active LPN license in the state of Wisconsin
Minimum of 2 years’ experience in nursing; case management experience preferred
Knowledge of healthcare coordination, community resources, and medical terminology
Strong communication, organizational, and critical thinking skills
Ability to work independently and collaboratively within a team environment
Must have valid drivers license (must be able to drive to 9 locations- Milage reimbursement available).
