Join us on this meaningful journey where your skills, compassion, and dedication will make a remarkable difference in the lives of those we serve.How you'll contributeProvides aftercare coordination for patients who need ongoing medical/psychosocial care following dischargeCreates and maintains up to date aftercare coordination community resourcesDisplays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist.Provides resource information to patients and families as appropriate for future care needs.Actively communicates with clients, families, aftercare providers, and referral sources.
