Overview
You will work closely with medical providers, a psychiatric consultant, and the primary care team to provide evidence-based interventions, facilitate patient engagement, and monitor treatment progress. Your responsibilities will include conducting assessments, offering brief therapeutic interventions, educating patients about mental health and substance use disorders, and coordinating and updating care plans to achieve optimal patient outcomes. By fostering strong therapeutic relationships and providing compassionate care, you will help empower patients to achieve their mental health goals and improve their overall quality of life.

The Behavioral Health Care Manager works under the direction and supervision of the PCP and is responsible for ensuring the program goals are met for clients enrolled in our Certified Behavioral Health Clinic, ensuring compliance with Illinois Department of Healthcare and Family Services (HFS) requirements and guidelines. This full-time role involves providing clinical guidance to staff, ensuring the delivery of high-quality patient care, and maintaining compliance with state and federal regulations.

Key Responsibilities

  • Clinical Care:
  • Deliver behavioral health therapy to patients/clients for up to 15 hours each week, depending on clinic needs.
  • Customer engagement and education.
  • Developing, reviewing, and updating person-centered care plans.
  • Updating the care plan for all COCM members
  • Administering validated screening tools (e.g., PHQ-9 or GAD-7) for each client at least monthly.
  • Delivering evidence-based brief interventions.
  • Maintaining the patient registry.
  • Collaborating with team members.
  • Develop with leadership the intake process and Standard Operating Procedures for Behavioral Health
  • Program Development and Management:
  • Monitor and enhance the continuum of care for patients.
  • Support non-clinical staff and facilitate non-clinical programming as needed.
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant, and who may need more intensive or specialized mental health care.
  • Staff Development:
  • Identify training opportunities and collaborate with HR to organize staff training sessions.
  • Administrative Duties:
  • Ensure compliance with documentation timelines (e.g., within 24 hours of service).
  • Partner with the Clinical Director to maintain clinical policies and procedures aligned with organizational and state standards.
  • Update the care plan monthly for all enrolled members.
  • Organize care rounds for COCM, Minimum 5 % of the enrolled
  • Track progress and Key performance indicators for BH
  • Collaboration and Communication:
  • Consulting weekly with the psychiatric consultant.
  • Collaborating with team members, including facilitating communication between the PCP and the psychiatric consultant
  • Participate in weekly interdisciplinary meetings to address clinical and operational needs.
  • Provide crisis intervention and consultation for clients and families as needed.
  • Facilitation of referrals to social services.
  • Partner with HR, Clinical Managers, and other departments to address staffing and clinical concerns.
  • Oversee the development, implementation, and evaluation of behavioral health programs and services.
  • Provide clinical supervision to staff members, ensuring adherence to best practices in psychotherapy and applied behavior analysis.
  • Collaborate with multidisciplinary teams to create comprehensive treatment plans for clients.
  • Manage crisis situations effectively, employing appropriate intervention strategies.
  • Ensure compliance with all regulatory requirements related to behavioral health services.
  • Maintain accurate documentation and coding for all client interactions, utilizing ICD-10 and ICD-9 standards.
  • Provide training and support to staff on working with individuals with developmental disabilities.
  • Engage in community outreach efforts to promote awareness of available behavioral health services.

Experience

Qualifications and Education Requirements

  • A bachelor’s or master’s degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field
  • A bachelor’s degree in any field with two years of documented, supervised clinical experience in a behavioral health setting
  • Registered Nurse (RN).
  • Current Illinois licensure as LCSW, LCPC, Licensed Clinical Psychologist, or equivalent required.
  • Additional licensure in Tennessee, Texas, California, or New Mexico preferred.
  • Experience in inpatient and outpatient psychiatric or addiction treatment preferred.
  • Experience as Behavioral Health Care Manager for COCM ( preferred).
  • Ability to work weekends/evenings as needed

Knowledge, Skills, and Abilities

  • Strong understanding of behavioral health treatment modalities, including addiction and family dynamics.
  • Knowledge of DSM-5 and experience with screening for common mental health diagnoses
  • Experience and comfort in interactions and professional dialogue with clinicians
  • Proficiency in electronic medical records (EMR) systems.
  • Strong leadership, conflict resolution, and decision-making skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Proficient in Leveraging Virtual Tele Platforms
  • Commitment to maintaining patient confidentiality in compliance with HIPAA.

Physical Requirements

  • Prolonged periods of sitting, standing, or walking.
  • Ability to lift up to 15 pounds.
  • Clear verbal communication and auditory perception.
  • Ability to perform repetitive tasks such as typing for extended periods.

Position Summary:

The Behavioral Health Care Manager (BHCM) plays a crucial role in supporting integrated behavioral health care within the primary care setting.
