Ultra Health Providers is seeking a compassionate Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to support residents in skilled nursing facilities. You'll deliver personalized therapy, support trauma recovery, and collaborate with medical teams to improve emotional well-being and outcomes for aging adults.

What You’ll Do

Provide individual therapy & trauma-informed care

Create treatment plans & maintain ICD-10 records

Coordinate care with medical staff

Support behavioral health & emotional wellness

Minimum Qualifications

  • Active LCSW license in Tennessee
  • Experience with geriatric or adult populations
  • Skilled in psychotherapy & clinical documentation


Preferred Skills:

  • Ability to work independently
  • Dependable and reliable
  • Knowledge of working with individuals with developmental disabilities
