Job Overview
We are seeking a qualified and dedicated Care Coordinator to oversee and ensure the effective planning, implementation, and evaluation of services for individuals receiving 245D and PCA/CFSS services. The Care Coordinator will play a key role in maintaining compliance with Minnesota DHS regulations and supporting our Direct Support Professionals and administrative staff.
Responsibilities
- Coordinate and oversee the development and implementation of person-centered support plans (CSSP/Addendums)
- Monitor and evaluate service delivery to ensure compliance with 245D licensing standards
- Supervise, train, and support Direct Support Professionals (DSPs)
- Conduct quarterly visits to clients home, documentation reviews, and quality assurance audits
- Communicate effectively with individuals served, guardians, case managers, and interdisciplinary teams
- Maintain accurate records, service agreements, progress notes, and incident reports
- Participate in internal audits, DHS licensing inspections, and corrective action planning as needed
- Assist with onboarding and orientation of new staff
- Stay current on 245D regulations and best practices in the field

Qualifications
- Per MN 245D licensing requirements, candidates must meet Designated Coordinator qualifications which include one of the following:
- Bachelor’s degree in a human services-related field and 1 year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older
- Associate’s degree or 2-year certificate in a human services-related field and 2 years of full-time work experience
- Diploma or certificate in a human services-related field from an accredited postsecondary institution and 3 years of full-time experience
- Minimum of 4 years of full-time work experience with no degree, in providing direct support services to individuals with disabilities or persons age 65 and older

Additional Requirements:
- Valid Driver’s License and reliable transportation
- Ability to pass a DHS background study
- Excellent communication, organizational, and leadership skills
- Proficiency with documentation and computer systems
- Knowledge of Positive Behavior Support and person-centered planning preferred
- Bilingual preferred
