Job Title: Community Liaison - Private Pay Home Care

Salary: $90,000 to $120,000 Base + Commission - UNLIMITED EARNING POTENTIAL

Schedule: Full-Time (Monday – Friday) + on-call rotation

At Legacy Home Care, we’re expanding—and we’re looking for a proven prospector to identify, develop, and convert new business across the Twin Cities metro.

As a Community Liaison, you'll be the face of our services in the field—educating discharge planners, social workers, and providers while creating warm handoffs for families in need of home health support. You’ll have the autonomy to own your territory, and the backing of a well-established, mission-driven team.

What You’ll Do:

  • Identify and develop new referral sources through calls, drop-ins, and outreach
  • Initiate the referral cycle with hospitals, TCUs, clinics, and senior living partners
  • Deliver compelling presentations to social workers, discharge planners, and physicians
  • Act as a consultant to clients and families during referral follow-up
  • Attend community networking events to expand brand reach and visibility
  • Track pipeline activity, follow-ups, and relationship milestones with accountability

Who You Are:

  • A hunter by nature—you’re at your best when generating leads and breaking into new accounts
  • Experienced in healthcare sales, outreach, or business development
  • Skilled at building trust, identifying needs, and turning conversations into conversions
  • Organized, independent, and relentless in your pursuit of results
  • Comfortable with daily travel throughout the Twin Cities metro (company vehicle provided)
  • Confident communicator who thrives in person, by phone, and in writing
