Sierra Vista Behavioral Health Intervention Services Unit is seeking Per Diem Case Managers (AM/PM - Weekends). The Sierra Vista Hospital location, a 171-bed psychiatric care facility located in south Sacramento, is a full-service mental health facility and a center of support for patients and their families. Since 1986, we have offered our clinical expertise to those suffering from emotional, behavioral and addiction problems.

  • To provide quality case management and qualified professional support and services to inpatient social workers as assigned.
  • Works with local agency and service providers to facilitate sending and receiving collateral information, treatment referrals, and ongoing communications to support patient’s successful inpatient treatment and discharge.

Qualifications


Education: Master’s Degree from an accredited college or university in social work, counseling psychology, mental health or a related field & Transcripts.

Experience: Preferred one (1) year direct clinical experience in a psychiatric or mental health setting. Experience in patient assessment, risk and safety assessments, treatment planning, community resources, group and family therapy, family systems and communication, and coordination with external review organizations preferred.

Additional Requirements: Complete Crisis Prevention Intervention (CPI) and CPR certification within first 90 days of employment.
