About Job

Bright Path Home Care is a fast-growing, Medicaid-certified home care agency dedicated to helping individuals live safely and independently at home. We are currently seeking an experienced and connected Care Coordinator who has established relationships with care managers and service coordinators at Passport, MyCare Ohio, and other Medicaid managed care organizations.

Responsibilities:

  • Leverage existing relationships with care managers to facilitate referrals and authorizations for new clients.
  • Act as liaison between Bright Path and MCO case managers to ensure proper care coordination and follow-up.
  • Identify and outreach to care managers, service coordinators, and waiver teams to promote Bright Path's services.
  • Track and monitor new referrals, authorizations, and ongoing care plan updates.
  • Provide support to our internal client onboarding and care management teams.
  • Maintain accurate documentation of all outreach and care coordination activities.

Requirements:

  • Experience working with or for Medicaid managed care organizations (Passport, MyCare Ohio, Aetna, Buckeye, CareSource, Molina, etc.).
  • Established relationships with care managers, case managers, or service coordinators in the Ohio Medicaid waiver space.
  • Previous experience at a home care agency, MCO, Area Agency on Aging (AAA), or similar setting preferred.
  • Excellent communication and relationship-building skills.
  • Ability to work independently and meet outreach/referral goals.
  • Familiarity with waiver programs including PASSPORT, MyCare, and DODD is a plus.
