Care Coordinator- Residential Services Program: Full time 37.5 hours per week. The Care Coordinator is responsible for coordinating, assessing, and scheduling all admissions for the program. They are the primary contact for all referrals to the program and will be the primary point of contact for other providers, referral sources, and potential clients. The Care Coordinator will promote engagement and retention in treatment and facilitate an effective discharge plan whereby the individual successfully connects to the next level of care. The Care Coordinator will work with a multidisciplinary treatment team to help individuals engaged in the Residential Program to achieve their substance use recovery goals. This position may include weekend and evening hours.
Professional Requirements: Associate degree or two (2) years post-secondary education; or Equivalent combination of training & education. Two (2) – Five (5) years administrative experience in substance use and/or mental health. Bilingual Spanish preferred.
Staff Nurse – Full time 37.5 hours per week. Schedule determined by program need. Under the direct supervision of the Nurse Supervisor, the Staff nurse serves as a member of a multidisciplinary team, providing services to patients in our Medication for Opioid Use Disorder (MOUD) program. Schedule includes rotating weekends. Attendance is punctual and reliable, and necessary absences must be planned in advance. In addition, the ideal candidate must be willing to work in a high stress environment, must be able to manage multiple competing tasks, and demonstrate competency as a collaborative team member.
Professional Requirements: CT Licensed LPN or RN, CPR Certification, CT Driver’s License. Prior experience in the mental health and /or substance misuse field preferred.
Housekeeper- Residential Services Women’s Unit: Full time 37.5 hours per week. Under the general supervision of the Housekeeping Coordinator, the Housekeeper will be essential in maintaining cleanliness and order within common area, staff offices, bathrooms, and patient areas as directed within the Women’s unit of our Residential facility. This position will also clean and maintain other areas of the facility as requested. Patient confidentiality must be maintained at all times. Your relevant skills in janitorial and custodial work will enhance our team’s efficiency.
Professional Requirements: High School diploma or equivalent, CT Driver’s License. 1+ Years’ experience operating cleaning equipment. Knowledge of cleaning products, techniques and methods.
Licensed Clinician: Full time 37.5 hours + benefits package, sign on bonus. Schedule determined by program need. Provide group and individual counseling to patients receiving opiate replacement therapy through a comprehensive, recovery oriented and trauma informed treatment process.
Professional Requirements: Clinician 1 -Associate License issued by state of CT and must be within prescribed period of completing supervised hours. Clinician 2- Master’s Degree + licensed by the state of CT (LPC, LCSW, LMFT). Must have strong working relationships with community providers. Must have valid CT Driver’s license. Attendance is punctual and reliable, and necessary absences must be planned in advance. In addition, the ideal candidate must be willing to work in a high stress environment, must be able to manage multiple competing tasks, and demonstrate competency as a collaborative team member.
