Seeking a Medical Social Worker (MSW) in Oklahoma City, OK, for a full-time position with a Monday-Friday schedule and pay range of $28.00-$36.00/hour based on experience.
Responsibilities include managing care transitions, coordinating with care teams and providers, accessing community resources, supporting end-of-life planning, conducting assessments, and facilitating mental health and caregiver support.
Requires excellent communication, organizational, and interpersonal skills, along with proficiency in Microsoft Office.
Must have a Master’s in Social Work (MSW) from an accredited institution; LCSW certification and experience with the elderly are preferred. A valid driver’s license is required.
