Job Overview
The Social Services Coordinator is vital in overseeing and improving the organization's social service programs. The Social Service Coordinator manages and coordinates social services for individuals and groups requiring assistance. This includes needs assessments, service plan development, and connecting clients with appropriate resources and support. The goal is to enhance client well-being by addressing social, emotional, and financial challenges, ensuring access to necessary services and support.
Responsibilities
- Conduct client assessments to determine their needs and eligibility for services
- Develop and implement individualized service plans
- Coordinate and monitor the delivery of social services to clients
- Advocate for clients' rights and access to resources
- Collaborate with community organizations, healthcare providers, and other service agencies
- Maintain accurate and up-to-date client records and documentation
- Conduct follow-up visits and reassessments to ensure client needs are being met
- Provide crisis intervention and support during emergencies
- Organize and facilitate support groups and workshops
- Assist clients with applications for public assistance programs and benefits
- Participate in staff meetings, training, and professional development opportunities
- Educate clients and their families about available resources and services
- Prepare reports and maintain documentation for funding and compliance purposes Skills
- Experience in Social work or a similar role
- Knowledge of state and federal social service programs and regulations
- Experience in case management and client advocacy
- Ability to work with diverse populations
- Familiarity with community resources and service providers
- Experience with data management and reporting software
- Certification in social work or a related field
- Strong organizational and time management skills
- Ability to handle confidential information with discretion
- Excellent problem-solving and critical thinking abilities
- Proficiency in Microsoft Office and other standard office software
- Strong written and verbal communication skills
- Ability to work independently and as part of a team
- Empathy and a strong commitment to helping others
- Knowledge of community resources and social service programs
- Ability to travel within the community as needed
Job Type: Full-time
Pay: $31,000.00 - $36,000.00 per year
