The Lead Care Manager is responsible for providing holistic, person-centered care coordination combined with housing navigation and tenancy sustaining services for a manageable caseload of Medi-Cal members. This integrated role supports members' medical, behavioral, social, and housing needs through comprehensive assessments, individualized care planning, system navigation, and advocacy. The Lead Care Manager builds strong partnerships with landlords and community housing agencies to help members secure and maintain stable housing.
Key Responsibilities
Care Coordination
- Conduct comprehensive assessments of member clinical, behavioral, social, and housing needs.
- Develop, implement, and regularly update individualized care management plans with member and care team collaboration.
- Coordinate healthcare, behavioral health, and social services including referrals and care transitions.
- Advocate for members' access to necessary services and supports, emphasizing culturally competent, trauma-informed care.
- Maintain accurate and timely documentation and reporting in accordance with CalAIM, DHCS, and MCP requirements.
- Collaborate effectively with outreach specialists, compliance staff, and other internal and external partners.
Housing Navigation & Tenancy Support
- Conduct individualized housing needs assessments and develop housing support plans tailored to each member.
- Assist members with housing applications, move-in logistics, and connection to housing resources.
- Build and maintain relationships with landlords, property managers, and local housing agencies.
- Provide ongoing tenancy support to prevent evictions, including early identification of risks, mediation, and connection to rental assistance and legal aid resources.
- Educate members on tenant rights and responsibilities and support skill-building for housing retention.
- Conduct monthly member housing encounters and periodic home visits as needed.
- Participate in local Continuum of Care (CoC) meetings and housing coalitions to represent members' interests and stay current on housing resources.
Compliance & Quality
- Ensure compliance with all contractual, regulatory, and programmatic standards including documentation, billing, and reporting.
- Participate in quality assurance activities, case conferences, and staff trainings.
- Support data entry and accurate tracking of member and housing outcomes.
Qualifications
- High school diploma or GED required; Associate's or Bachelor's degree in social work, nursing, public health, or related field preferred but not required.
- Minimum 1 year of experience in care coordination, case management, housing navigation, or related social services with vulnerable populations.
- Knowledge of Medi-Cal, CalAIM ECM/Community Supports, housing systems, landlord-tenant laws, and supportive housing best practices preferred.
- Strong communication, interpersonal, negotiation, and conflict resolution skills.
- Demonstrated ability to engage culturally diverse populations with sensitivity and trauma-informed approach.
- Proficiency with electronic health records and care management documentation systems.
- Valid California driver's license and reliable transportation required.
- Ability to work independently and as part of a multidisciplinary team.
- Willingness to participate in ongoing training and professional development, including motivational interviewing and SOAR certification if available.

- Combination of remote work, office time, and in-person member visits and housing site visits.
- Flexibility to accommodate member schedules and community meetings.
