Assists Director of Assisted Living and in Memory Care in developing and posting a work schedule for all caregivers. Manages the schedule throughout the month, documenting all changes on the schedule, and making arrangements for staff replacement during absences. Covers shifts as necessary as either a caregiver and/or Med Tech. Tracks caregiver attendance and takes appropriate corrective action as necessary. Assists in the completion of time records.
Coordinates services and assignments for and with caregivers assigned to the area. Assures caregiver skills matches the needs of assigned.
Communicates with the family and physician.
Coordinates move in of all residents as assigned. Reviews pre-admission appraisals and physician reports. Assures that all residents and /or potential residents meet the criteria for acceptance and retention.
Maintains resident file and assures that all information is accurate and up-to-date including physician reports, physician orders, and service plans.
Documents meetings, service plans and level of care evaluations on appropriate
Makes rounds daily go greet residents as assigned to evaluate services including care, food intake, housekeeping, activity participation, and apartment maintenance. Visits each resident in their apartment at least weekly but more often if appropriate or necessary.
Assures regulatory compliance including areas pertaining to residents, apartments, and common area.
Coordinate services with other departments including housekeeping, maintenance, activities, business office, and food service departments.
Participates in meetings that benefits coordination of services including safety committee, and any changes in level of care for our residents.
Coordinates discharge plans when appropriate. Coordinates move outs.
Investigates and completes reports regarding any incident.
May assist with touring prospects and/or taking phone inquiries as part of the backup sales team. Does so in accordance with established community procedures.
Follows all rules, regulations, policies, and guidelines as outlined in the Employee Handbook.
Performs all duties in a safe way. Participates in the safety program.
Performs other duties as may be required.
