The Social Worker is an integral member of the Social Services Department tasked with the psychosocial well-being of all residents. This role involves assessing and addressing the emotional, social, and discharge planning needs of residents, while fostering a supportive environment that promotes dignity, autonomy, and quality of life. The Social Worker collaborates closely with interdisciplinary team members, residents, families, and community partners to support seamless transitions of care and resident satisfaction.
Qualifications:
· Master’s Degree in Social Work (MSW) from an accredited institution.
· Minimum of 2 years of experience in a skilled nursing facility or related healthcare setting.
· Demonstrated knowledge of the aging process, geriatric mental health, and the psychosocial needs of older adults.
· Strong understanding of community resources, discharge planning, and case management.
· Excellent interpersonal, communication, and organizational skills.
· Ability to work collaboratively with residents, families, and interdisciplinary teams.
· Familiarity with regulatory requirements including CMS, HIPAA, and resident rights.
Responsibilities:
- Plans, directs and provides direct services for residents and their families.
- Plans and arranges discharge planning.
- Contributes to the Continuous Quality Improvement (CQI) of the Department.
- Participates in continued education and keeps abreast of changes in field/market.
