LPN Care Coordinator (1099 Contract | Fully Remote)

What You’ll Do:

Review patient EMR records to determine eligibility for care management services.
Make proactive patient outreach calls (monthly) to provide support, education, and resource connections.
Develop and maintain individualized care plans, ensuring patients receive the right care at the right time.
Monitor patient progress and collaborate with care teams to prevent gaps in care.
Ensure compliance with Medicare, HIPAA, and company policies.
Engage in high-volume patient outreach—this is a phone-based role requiring consistent daily patient interaction. You will be expected to meet daily performance targets, ensuring each patient receives a minimum of 20 minutes of care coordination per month.

What We’re Looking For:

LPN Compact License (or ability to obtain one if needed).
5 years of patient-centered care experience—strong clinical knowledge is a must!
Tech-savvy with EMR proficiency and comfortable working in a remote, digital environment.
Excellent communication skills—you'll be on the phone daily engaging with patients.
Self-motivated and detail-oriented—you take ownership of your work and stay on top of performance targets.
Fluency in Spanish is preferred—bilingual candidates are highly encouraged to apply!
