Tri County Care Management Organization (CMO) is a non-profit organization that provides care management (case management) services for children with complex behavioral, developmental or addiction challenges in Hunterdon, Somerset and Warren counties. We partner with families and communities to help youth succeed at home, in school and in the community.

We are currently seeking a Care Manager based in our Stewartsville office to provide care management (case management) services.

Responsibilities Include:

  • Provide intensive wraparound/case management services to families and children with mental health/ behavioral/developmental challenges.
  • Advocate and help find resources for youth to access formal and informal services and be a liaison for care services.
  • Attend meetings at homes, schools and other services with the youth and families.
  • Document and follow up on plans of care.

Position Requirements:

  • Bachelor’s degree in social work, psychology or related discipline
  • At least one (1) year of experience in a child service system environment
  • Experience with DD/ID population preferred
  • Strong interpersonal and organizational skills
