About Job

For more than 30 years, Henry Ford Hospice has provided care for our patients and their family/caregiver unit at end of life with a focus on quality and helping patients live every day the best way possible. Our team of expert physicians, nurses, social workers, chaplains, home health aides, registered dieticians and bereavement specialists work together to provide professional, comprehensive, compassionate, patient centered care.

GENERAL SUMMARY:

  • Responsible for the provision of social work services to patients and their families.

EDUCATION/EXPERIENCE REQUIRED:

  • Masters of Social Work degree required.

  • Must be a Licensed Master's Social Worker (LMSW).

  • ACSW and previous experience in a healthcare setting preferred.

  • Demonstrates understanding of the impact of involvement, personal attitude toward illness, handicaps, death and loss, on the quality of patient care and staff effectiveness.

  • Knowledgeable of the basic medical procedures, terminology, etiology and disease process associated with the illnesses of the client population.

CERTIFICATIONS/LICENSURES REQUIRED:

  • Licensure in the State of Michigan as a Social Worker (LMSW).
