Job Description:

The Anne Arundel County Department of Aging and Disabilities is seeking a geriatric social worker/case manager under the purview of the Department of Aging and Disabilities. The goal of the case manager is to identify and address the needs of County residents that will assist them to age in place in the community. The objective of these teams is to connect residents to resources; reduce the risk of emergency room visits, hospital admissions, or long-term care because they are not connected to County resources; and/or provide additional supportive services to clients as needed.

Duties Include:

● Coordinate assessments, resources, and supports to clients with the goal of improving client quality of life and outcomes

● Provide feedback to Department leadership and partnered liaisons in regards to target population in order to enhance care coordination and client satisfaction

● Track outcomes of referrals and treatments of targeted population

● Mobile, in-home support and care coordination will be provided to the clients to administer assessments, interventions, and future planning, as well as connect clients to resources and services

In addition, the Geriatric Social Worker will:

● Identify preventive and/or protective social services and facilitate the delivery of those services to vulnerable adults and their families

● Assure each client is provided with the necessary care and services to attain or maintain the highest level of well being.

● Coordinates and communicates efforts within their team

● Consider clients and their needs as a whole person, being willing to consider their health, mental health, environment, etc.

Minimum Qualifications:

● Possesses a Master’s Degree or higher in Social Work

● Current license as an LMSW in the state of Maryland

● Experience in working with older adults

● Demonstrated strength in identifying the medically related social and emotional needs of the adult geriatric population and to advocate and intervene on their behalf.
