  • Position Summary:Effectively utilizes hospital resources in the development of care, discharge planning and Utilization Review.Utilization Management performs prospective, admissions, observation and concurrent review according to standard policies and procedure.Utilization Management reviews and analyze processes designed to ensure appropriate utilization of medication, blood products, operative and invasive procedures, and use of resource intensive diagnostic testing and treatment.Discharge Planning assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.Discharge Planning assists with LTC, SNF, Rehabilitation, Behavioral/Mental Health, Home Care, DME, transportation and other placement planning needs.Discharge Planning assists with interactions with CPS, APS, Adoptions and end of life planning and completion of Advance Directives.Discharge Planning is a liaison with third-party payers and government programs to access special services for patients.Qualifications:Minimum Qualifications:
  • ADN or BSN required.
  • Current licensure applicable to professional affiliation required.
  • Certification by the National Association of Healthcare Professionals or InterQual is desirable.
  • Current New Mexico licensure as a Registered Nurse.
  • Certification as CCM or ACM desired.
