As a Nurse Care Manager, you'll collaborate with internal and external treatment team members to deliver comprehensive care for clients who have or are at risk for complex medical conditions. Key Responsibilities
Develop and implement personalized treatment plans for Healthcare Home enrollees with co-occurring chronic diseases
Facilitate health education groups, utilizing advanced knowledge of complex medical conditions
Manage hospital admissions and discharges for Healthcare Home clients
Provide education and training to patients on complex health conditions, medical diagnoses, treatments, and medications
Implement and utilize health technology programs such as Cyberaccess, Care Manager, and IntellectAbility
Monitor and report performance measures and outcomes using sophisticated nursing information systems
Collaborate with multidisciplinary treatment teams to ensure comprehensive patient care
Develop and implement wellness and prevention initiatives
Participate in required meetings and quality assurance procedures
Adhere to Brightli's Operational Guidelines and professional code of ethics
Education And/or Experience Qualifications
Active RN or LPN license
Experience with the IDD (Intellectual and Developmental Disabilities) population preferred
Additional Qualifications
Knowledge of chronic disease processes in children and adults
Excellent written and verbal communication skills
Ability to implement innovative systems and make decisions based on critical thinking and nursing process
Successful completion of background check including criminal record, driving record, and abuse/neglect screening
Current driver's license, acceptable driving record, and current auto insurance
Must complete New Hire Orientation at the beginning of employment
Must complete all training requirements including Relias at the beginning of employment and annually thereafter
