Social Services Assistant
Sterling Care Riverside s located in Belcamp, which is now more commonly known as Riverside. Sterling Care Riverside is a premier healthcare facility that bridges the Riverside community to all areas of Harford county for neighboring residents. In keeping with our organization’s goal of improving the lives of the Resident’s we serve, the Social Services Director ensures that the medically related emotional and social needs of the Residents are met and maintained on an individual basis and in accordance with current federal, state, and local regulations.
Some of the job requirements are:
1. Responsible for keeping up-to-date evaluation documentation on each Resident’s activities at the facility which complies with Federal, State, and Local regulations.
2. Develops a comprehensive social history and psychosocial assessment that includes the Resident's problems and strengths and preferences, including cultural, religious, and ethnic background and implications for the care plan.
3. Establishes and develops a comprehensive discharge plan in collaboration with Residents, Residents' families, multidisciplinary staff, and referral agencies.
4. Orients Residents and families to the facility, its services, its service limitations and Residents' rights.
5. Attends scheduled care conferences and identifies and interprets psychosocial needs of Residents for inclusion in the care plan, addressing in particular issues of schedule, treatment and environment that express and reinforce individuality and identity.

Two years’ experience in a social worker role, preferably in a health care setting
Bachelor’s degree in social work or similar field.
