Healthy Living Intensive Case Manager
Hours: Monday - Thursday
Status: Full-Time - Non-Exempt
Reports to: Adult Housing Programs (AHP) Supervisor
About the Department:
The New Beginnings Intensive Case Manager (ICM) provides permanent supportive housing to chronically unhoused adults without children (AWOC). Support participants in overcoming challenges and building a pathway to a better quality of life, foster self-sufficiency, and stability.
Job Summary:
The Healthy Living Intensive Case manager reporting to the AHP Supervisor provides direct intensive case management services to medically fragile, chronically homeless adults.
- Conduct initial interview/intake, and client needs assessments
- Provide case management and supportive services to clients moving into long-term housing, to promote transition from unhoused to residential stability
- Create individualized case plans with each adult member of the household, based on the client's needs, to achieve self-sufficiency
- Review the client's treatment plan monthly, update the client's case plan, and track outcomes
- Connect clients with wrap-around services through other programs offered at HELP, including Workforce Services Job Readiness Workshops, Energy Assistance Program, holiday programs, etc.
- Connect clients with mainstream programs, including but not limited to, Medicare, and SSI/SSDI
- Provide clients with referrals and resources for other social services and partner agencies
- Communicate effectively with individuals from a variety of social, cultural, and economic backgrounds
- Meet with clients monthly and complete income calculations and rent requests before the 1st of the following month
- Conduct weekly/semi-monthly home visits
- Provide transportation to clients to ensure that all appointments are kept
- Enter case notes and services daily into HMIS, reflecting activities of the client's progress
- Attend monthly case conferencing meetings with Managed Care Organization (MCO) to discuss and review client progression
- Always maintain audit-ready files
- Maintain clear boundaries with all clients and providers
- Must never allow your or the organization's ethical standards to be compromised
- Facilitate Life Skills Training sessions for clients on-site 2-3 times per quarter
- Attend meetings and trainings, as required
- All other job-related duties as assigned by management for needs of the business
The ideal candidate will possess the following knowledge, skills, and abilities:
- Knowledge of local social service agencies and programs, as they pertain to the local unhoused population
- Knowledge of applicable statutes, ordinances, and codes
- Knowledge of social service work, and interviewing/referral techniques
- Experience and understanding of unhoused issues
- Understand and follow complex instructions and procedures
- Ability to analyze situations accurately and adopt an effective course of action
- Communicate effectively with persons from a variety of social, cultural, and economic backgrounds, and possess a demonstrated understanding of and sensitivity to culturally diverse populations.
- Effective written, verbal, and listening skills
- Ability to independently initiate and maintain effective working relationships and respond to correspondence from all business contacts
- Ability to be flexible and quickly adjust to shifting priorities
- Organize work schedule and budget time
- Understand and follow all agency policies and procedures
- General computer knowledge, and the ability to use Microsoft Office software
- Ability to work flexible hours, as required by the client caseload.
- Ability to perform accurate mathematical equations, calculate client's rent and utility costs and request funding to pay those bills
Required Qualifications:
- Proof of Highschool Graduation or GED
- Active Nevada Drivers License with clean MVR
- 2 years' experience with data entry, compliance, or related field
- Previous experience working with unhoused population
Preferred Qualifications:
- Bachelor's Degree in Social Work or related field
