Medi Home Health & Hospice, a proud member of the Medical Services of America, Inc. family, is excited to invite dedicated social workers to apply for our Full-Time Hospice Medical Social Worker position. We are looking for compassionate individuals to join our dynamic team serving patients and families.

Key Responsibilities:

  • Provide social work services in alignment with the physician's plan of treatment.
  • Collaborate with the interdisciplinary team to conduct assessments and support patients and families effectively.
  • Assess for normal and complicated grief; provide bereavement counseling as needed.
  • Act as a liaison with local community resources to enhance patient support.

Qualifications:

  • Master's or Bachelor's degree in social work from a program accredited by the Council of Social Work Education.
  • Current licensure as a social worker in the state of NC.
  • Preferred minimum of one year of Hospice experience.
  • Valid Driver's License is required.
